A licensed, professional social worker acts as a liaison between the patient, family, physician, ancillary health care staff and community agencies in an effort to coordinate appropriate discharge plans from the Medical Center. Social Services also assists patients and families in coping with psychosocial issues that relate to illness or hospitalization.
